Signing up is simple
There are many ways to sign up for MyChart. If you use FollowMyHealth,?or the NextGen PXP patient portal, you will have to sign up for MyChart to stay connected with us.
Once you are registered with an 橙子视频app MyChart account, you can sign in and view your health information at any time on your computer or smartphone. For easy access, download the MyChart app in the app store for your iPhone or on Google Play for your android.
You may get an enrollment email with a sign-up link. This link is your access to register for a MyChart account. You may also receive an activation code via email.?
In-person
If you don't have an email address and wish to register for a MyChart account, you can sign up with our reception staff in the office. No appointment needed.?

What to expect
For your first appointment after we’ve moved to Epic, you can expect to:
- Re-sign consent, access, payment authorization, and HIPAA forms
- Provide your photo ID and health insurance cards
- Receive assistance signing up with Epic if you haven’t already registered for your new account

Progress notes are shared with patients, unless otherwise specified by providers.
All diagnoses are shared with the patient unless they are manually unshared by a clinician.
All results are released to MyChart.
All tests are released immediately after they are resulted. Providers, using discretion, may or may not, delay releasing results on a patient-to-patient basis.
All medications on a patient's medication list are visible in MyChart.
Patients can make payments, view their statements, and/or contact the billing departments about questions from MyChart.
Patients and proxies can schedule visits with Primary Care Physicians or other select providers they have existing relationships with.
Patients can give proxy access to their account to other family members.
Proxies have full access to child's chart until the child turns 12 years old. When the child turns 12, proxies can review insurance coverages, make payments, update demographics, access letters, and update end-of-life planning information.
MyChart is also available in Spanish. Note that while most text within MyChart is translated, clinical content and messaging remain in English.
Patients can log into the MyChart website or MyChart app to complete eCheck-in and join their scheduled video visit.
Patients can send their provider/clinic messages concerning questions about non-urgent medical issues, prescriptions, test results, or any other questions they may have.
Patients receiving care in the Inland Empire and San Diego County can log in in to MyChart
